Illinois State Health Record Requirements by Grade Level - Kindergarten, 2nd, 6th, 9th, 12th. 

Also requirements for new students from out of state, students transferring from other Illinois schools, and sports physicals. 

 

Kindergarten

The State of Illinois requires children entering kindergarten to have the following:

  • A physical exam 

    • The physical exam must be completed within one calendar year prior to the date of entry. The exam must be on the required “Certificate of Child Health” form. This exam must include a list of up-to-date immunizations and medical history completed and signed by a parent or guardian. 

  • Immunizations must meet Illinois Public Health Guidelines.

    • Kindergarteners are required to have proof of four or more doses of DTP/DTaP, four or more doses of Polio, second MMR & second Varicella.

  • Eye Exam

    • The eye exam must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year.

  • Dental Exam

    • The dental must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

  • Lead Screening

    • The lead screening is required one time prior to entering kindergarten for students six years or younger.

  • A copy of the student’s original Birth Certificate (not the hospital copy)

  • A copy of the student’s Social Security Card


The deadline for the physical exam, up-to-date immunizations, and an eye exam is October 15. If your student is not found compliant with Illinois School Code mandates for physical examination and immunization requirements, the student will NOT be allowed to attend school after October 14 of the current school year until items are received by the school.


2nd Grade

The State of Illinois requires children entering 2nd grade to have the following:

  • Dental Exam

    • The dental must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.


6th Grade

The State of Illinois requires children entering 6th grade to have the following:

  • A physical exam 

    • The physical exam must be completed within one calendar year prior to the date of entry. The exam must be on the required “Certificate of Child Health” form. This exam must include a list of up-to-date immunizations and medical history completed and signed by a parent or guardian. 

    • Please Note: The IESA/IHSA sport physical forms MAY NOT be used as the required school health exam. However, a physical exam completed on the “Certificate of Child Health” form may be used as a sport physical.

  • Immunizations must meet Illinois Public Health Guidelines

    • 6th graders are required to have proof of one dose of Tdap and one dose of MCV4 (Meningococcal Conjugate Vaccine) on or after the 11th birthday.

  • Dental Exam

    • The dental must be performed by a licensed dentist and submitted prior to May 15 of the school year. Dental exams must be completed within 18 months prior to the May 15 deadline.

9th Grade

12th Grade

The State of Illinois requires children entering 12th grade to have the following:

  • Immunizations must meet Illinois Public Health Guidelines

    • Proof of two doses of MCV4 (Meningococcal Conjugate Vaccine) vaccine received on or after 11 years of age. All twelfth graders will need to show proof of receiving two doses unless the first dose was administered after 16 years of age, in which case only one dose would be required at 12th-grade entrance.


The deadline for the up-to-date immunizations is October 15. If your student is not found compliant with Illinois School Code mandates for immunization requirements, the student will NOT be allowed to attend school after October 14 of the current school year until items are received by the school.


New Students from Out of State

The State of Illinois requires children entering an Illinois school for the first time to have the following:

  • A physical exam 

    • The physical exam must be completed within one calendar year prior to the date of entry. The exam must be on the required “Certificate of Child Health” form. This exam must include a list of up-to-date immunizations and medical history completed and signed by a parent or guardian. 

  • Eye Exam

    • The eye exam must be performed by a licensed optometrist within one year prior to the first day of school and submitted before October 15 of the school year.


The physical exam and proof of up-to-date immunizations are due within 30 days following registration. If exams are not submitted within the 30 days, the student will be excluded from school per Illinois Law until items are received.   


Students Transferring from Other Illinois School 

The State of Illinois requires students transferring from other Illinois school to have the following:

  • Transfer of current Illinois school health records

    • This includes a physical exam with up-to-date immunizations, dental exam and eye exam. 

  • School health requirements for grade levels as noted above.


Sport Physicals

A yearly sports physical is required for all athletes to participate in school-sponsored athletic programs.

  • Sports physicals are required for grades 5th, 7th, 8th, 10th, 11th, 12th

  • 6th and 9th graders do NOT need a separate sports physical. The required physical exam for 6th and 9th graders can also be used as a sports physical. * A sports physical CANNOT be used as the required 6th and 9th grade physical exam.*

  • Sports physicals are good for 395 days from the date of the physical and must be current for the entire length of the sports season.
